Intake Form

To make a referral please complete the form below. Referrals will then be screened by one of our mental health professionals. Referrals do not guarantee counseling services, additional intake paperwork is required before counseling sessions will begin.

One-on-One Counseling

Provide short-term counseling to Club members to develop social-emotional skills

Social Emotional Programming

Mighty Minds: A social and emotional wellness group Positive Action: A group based on the intuitive philosophy that we feel good about ourselves when we do positive actions

Prevention Programming

Students Against Destructive Decisions (SADD) empowers and mobilizes students and adult allies to engage in positive change through leadership and smart decision-making

Behavioral Support

Our social-emotional staff helps deescalate and reintegrate club members into programs and may use behavior plans to set clear expectations & goals for club members who may need additional support
